New Patient Registration

For patients that are new to our practice, we’ll need to gather some information about you. This includes demographic information as well as your medical history and current medications, etc. The easiest way to do this is to register online through our secure patient portal. Click here to begin.


If you prefer, you may also download our patient registration forms below and bring those in with you for your first visit.


When you come to our office for your initial visit, please bring with you a photo ID and your insurance card (if you have insurance).


We are very excited that you have chosen us for your medical care and look forward to seeing you in our office soon!

Appointments

We schedule patients by appointment. If you are unable to make a scheduled appointment, we ask that you please contact our office to reschedule as soon as possible. We prefer at 24 hour notice. This will give us the opportunity to accommodate another patient who could use your appointment time.


Although we do not encourage “walk-ins,” we do want you to call our office when you have an acute issue that needs treatment same day. We will always do our best to accommodate you quickly.

Financial / Insurance

We accept most major health insurance plans. Please contact our office to confirm that we participate in your plan prior to your initial visit. Any co-payments, deductibles and non-covered services are due at the time of service.


For your convenience, we do accept most major credit/debit cards, checks and cash.

Privacy

We value the privacy of our patients and we follow HIPAA guidelines for protecting your health information.

Prescription Refills

When you need a prescription refilled, please call our office with the following information:

  • Your pharmacy name and phone number
  • Name of your prescription medication and dosage


We ask that you give us 24 hours to authorize your prescription refills. As a general rule, we do not refill prescriptions after hours unless it is an emergency.
